Briggs “Steps Up” Commitment to Arm Patients with Ability to Independently Manage Their Health Care

with VNA First/Innovative Healthcare
Solutions to exclusively offer Step by Step patient education materials. These
materials have been developed to empower patients to best manage their diseases
one step at a time.
“Step by Step” patient education works to progress patients from dependant care to independent disease management. These standardized materials allow clinicians to better educate each patient on his or her specific disease and the care needed as well as provide a benefit to the patient with simple terms and procedures.
Designed with the patient in mind, the booklets and tearpads provide a thorough resource presented in an easy to understand format with colorful illustrations and simplified language, available in English and Spanish. The information is divided into “steps” so it doesn’t become overwhelming for patients and allows them to move at their own pace. All of the materials may also be customized to the individual patient and circumstance with fill-in-the-blanks and check boxes.
“Patient/caregiver education was used to help reduce hospitalizations by 48% of the participants of our study,” said Cyndi Rohert, RN, BSN, CHPN, CRNI, Briggs clinical consultant for home care and hospice. “All patients admitted home care should receive information about preventing the deterioration of their condition. Standardized teaching tools should be used whenever possible.”
